Paper consultation forms and treatment records are stored securely and only accessible to me.  Where records are held electronically, they are stored on password protected devices with appropriate security measures in place to protect personal information.  Reasonable measures are taken to prevent unauthorised access, loss, misuse, or disclosure of personal information.

Your information will remain confidential and will not be shared with third parties except:

How Long Information Is Kept.
I retain records only for as long as necessary to meet professional, insurance, legal, and business requirements.  Adult treatment records are retained for seven years following your last treatment.  For clients under the age of 18 records are retained until the clients reaches the age of 25. Where treatment is provided at the age of 17 records are retained until the client reaches the age of 26.

Therapist Rights
If you don’t agree to your therapist keeping records of information about you and your treatments, or if you don’t allow them to use the information
in the way they need to for treatments, the therapist may not be able to treat you.  Your therapist must keep your records of treatment for a certain period as described above, which may mean that even if you ask them to erase any details about you, they might have to keep these details until after that period has passed.  Your therapist can move their records between their computers and IT systems without your permission if your details are protected from being seen by others


Data Protection Complaints
If you have concerns about how your personal information is handled, please contact me in the first instance.  I will acknowledge your complaint and aim to respond within 30 days. If you remain dissatisfied, you have the right to complain to the Information Commissioner's Office (ICO). The ICO can be contacted via their website at www.ico.org.uk.
